We do not participate with any Medicaid plans. Our practice requires that all patients keep a valid credit card on file. In the event that a patient has a co-insurance, deductible, a change in co-pay amount or your visit is not covered due to a pre-exisiting medical condition or for any other reason, your card will be charged. If the charge is more than $40, we will contact you prior to charging your card.
If you have an HMO please elect us prior to your visit. For Keystone plans, our Philadelphia office HMO ID is 3436252001. For Keystone plans, our Main Line office HMO ID is Keystone provider number for our RIM Narberth office is 3436252002. For Aetna plans our Provider Identifier is 9727096 and our Primary Office # for Philadelphia is 381953. Our Aetna Primary Office # for Narberth is 780536.
